Two or three minutes at the end of class. It's a conversation rather than a form because the useful part is usually the second thing you say, and a form never asks for it.
This goes to Geoff with your name on it. Not anonymous, and it would be dishonest to say otherwise — the course knows who is signed in. Say the true thing anyway: it's read to change what happens next week, it's never a grade, and it doesn't leave the course. If something was bad, saying so is the useful move.
